The Generation Connect (GC) App platform helps people with dementia (PWD), their loved ones and healthcare partners mitigate or prevent behavioral and psychological symptoms of dementia via non-drug interventions.

Inputs

The GC App prompts users to add meaningful people, music, photos, life stories and topics for the PWD. Clinical teams customize app experience settings based on scope of care, stage of dementia, care setting, and caregiver relationships.

Interventions

The GC App prompts personalized, caregiver-led interventions based on care team input and usage data. Interventions may include offline activities (social outings, exercise, prayer, etc.) and online activities (family communications, reminiscing, music, etc.).

Outputs

The GC App checks in with primary caregivers and tracks engagement trends related to the care recpient’s social wellbeing and purpose. Clinical teams may also use assessment tools to track behavorial symptoms and caregiver wellbeing.

Care recipient trends and outcomes create a real time feedback loop, triggering personalized suggestions for engagement and care planning.
